Review a Tell Your Story Record

When a safety story is put in PeopleSafe a Story record is created. This article explains how to do a  review of a Tell Your Story Record. The goal is to learn as much as possible about the story so that when you come to deciding how to make work safer, you'll have all the information in front of you. 

Before you do a review of a Story, you'll need:

  • Security clearance of Manager Users of a person involved in the story or PeopleSafe Coordinator
  • Access to the people who were involved in the story
  • Any additional information (ACC forms, Doctors certificates, etc)  

Review a Safety Story

Step One - Get to the Story Record:

There are two options:

  1. Click on the link in the email you received from PeopleSafe alerts. If you are already logged in to PeopleSafe you will go straight to the Story Review Task screen. If not, you'll be sent to the login page, login & you'll be taken to the Story Review Task screen showing the task, including who the task has been given to. Now go to No. 4
  2. To get to the list of Stories needing review login to PeopleSafe, click the box labelled Stories needing review and you'll be taken to the Tasks(Storyonly)ToDo screen, this is a pre-filtered smart list displaying the tasks for stories needing review.

    Note: PeopleSafe Coordinators will see all stories, Manage Users will only see stories where Users they manage told the story or were added as a person involved with the story.
  3. Select the Story you want to reviewclick the View Button. This will take you to the Story Review Task screen. This shows full details of the task including who the task has been given to. You will also be given options to edit the task. 

  4. Click the Unreviewed Story link to view the full story record and find out who will need to be involved with the review.

  5. To see all details of the Story record continue to scroll down the page.
  6. Click Options, then click Review to start the story review.This takes you to the Reviewing a Tell Your Story Report screen where you begin the review.


Step Two - Start the review:

Note: The original story record is unchanged by a review. A review creates a new record that includes any extra details.

The first screen is the same for all stories. The screens which follow change, depending on the content of the story.

The Review details are only saved once you get to the Completion screen at the end and click Save.

Click the Back Button to go back to a previous screen and the Next Button to move forward.

  1. Review the details that have been added for each question.  Enter any new information as you go. 

  2. Confirm if these  injuries actually happened - check the relevant boxes
  3. What was involved in the Story? - check the relevant boxes.  If you want to add details about them use the text box provided  


RISKS screen

Note: Once all the facts are known, any risks can now be assessed. This is critical to make sure you learn from a story.

If you've decided risks were involved there are two categories of risks:  Risks you already have safety plans for and new risks. 

If you click YES to the first question, further questions appear and  more questions about those risks will come up  later in the review process. This will include an opportunity to add the details about those risks. If you click No, no more risk questions appear.

To check if you have a safety plan for risks that you've identified,  use the (check here) link on the Risks screen and a page will open in a new browser tab showing all your current safety plans.


Step Three - Review Complete:

  1. The final screen displays  Well Done That Review Is Sorted  showing your review is successful. A number # is now allocated to the Story.
  2. The Completion (Success)  Screen may also tell you, you might need to give some tasks to people in your organisation.

  3. Click the Add Tasks button if you want to give tasks that relate to the reviewed story.  The New Task for Story #   screen appears.

Common Questions 

How do I find if a risk has a Safety Plan in place? 

  1. Scroll through the Safety Plan screen to check if your risks appear in a plan. To view a Safety Plan select the plan and click on View. This will give you the full details.
  2. To return to the Reviewing a Tell Your Story Report screen click  the arrow at the top left of the window.

What email alerts might be created at the end of the Review process?

If any new risks have been added this will create two email notifications:

  1. You have an un-reviewed risk
  2. You have a task,to review the new risk.

If a new risk appears "serious" an alert is also created to say a Safety Plan is needed. If tasks are added at the “got tasks” Completion screen,an alert will also be sent for every task that is added. This will go to the people you have chosen to do the task/s (even if you’ve added the task for yourself).

What do I do if I don't have all the information to complete the review in one go?

Complete the review with the information you have. You can always go back and  make updates later  when more information is available.

Can I add tasks at a later date to a story?

Yes - go to the Story record >Options menu and select Add Task -  Add a new task.

Can I add a new person or another injury to the story?

No - but you can record a new story that includes that person or injury.

Do the different colours on the dashboard have any significance?

  • Green = sorted
  • Orange = becoming due within 30 days
  • Red = overdue

How come some people can see more stories than others?

PeopleSafe Coordinator will see all stories,  Manage Users access is restricted to stories that involve Users they manage.
